DIABETES MANAGEMENT PROGRAM

This program identifies participants with poorly controlled diabetes and hypertension and invites them to participate in the program to ensure stabilization and improvement of their health. By joining, participants can start making positive changes in their lifestyles and help them better manage their disease.


Proyecto Juan Diego community health workers (CHWs) are trained to educate participants about diabetes and hypertension management and help them define their goals to help improve and control their diabetes.

Each CHW has a participant load of 50 to 60 people at various levels along the continuum, often with complex and interrelated problems. Their listening, relational, and observational skills are key to the success of the participants.



All participants are required to attend Diabetes Self-Management Education Classes (DSME) provided by the CHWs at Proyecto Juan Diego. Class topics include: What is Diabetes?, Nutrition, Medications and Physical Activity, Complications, Stress and Depression, Self-Management, and Blood Pressure.


 

 

DIABETES PREVENTION
PROGRAM

GLB and Prevent T2 are programs for overweight adults who are at risk for developing diabetes. The program provides education, encouragement, and the tools necessary to help individuals reach their healthy lifestyle goals. This program is designed for non–diabetic, overweight individuals aged 18 and older who have pre–diabetes and/or metabolic syndrome.


GLB is a one-year program and consists of (12) weekly core sessions, (4) bi-weekly transition sessions, and (6) monthly support sessions. Prevent T2 is also a one-year program that starts off with (16) weekly sessions and then transitons into monthly sessions. The goal of these programs is to help participants lose 5-7% and to increase the physical activity by 150 min per week.



Our diabetes prevention coaches educate and encourage participants to start and continue with their new lifestyle change by monitoring their calorie intake as well and their amount of physical activity. GLB coaches will have weekly talks one on one with participant.



Every week the participants weight will be monitored, and every 3 months participant will have a follow up of their measurement and a1c we will also monitor their fruit and vegetable intake this will include incentives for attending the follow up sessions.


 

 

SENIOR ADULT SERVICES


Regular exercise can help older adults stay independent and prevent many health problems that come with age. P.E.S.A.S (Prevention. Ejercicio. Suave. Activa. tu Salud) focuses on low-impact exercise and is specifically designed for anyone over 55+ who would like to stay active without getting injured. Classes are every Wednesday (Zoom and In-person) and Friday (Zoom only) from 10 a.m. to 11:30 a.m.



Generation WE is a program that focuses on establishing family relationships to connect with family members across generations (grandchildren and grandparents) through different quality-time activities. Activities are every 2 weeks for a lapse of 6 months.


 

 

GROUP EXERCISE
CLASSES

Engaging in physical activity is key to improving the health of those we serve. Proyecto Juan Diego offers 1-hour group exercise sessions twice a day, at no cost, to the community. Classes are Monday thru Friday from 8:00am-9:00am and evenings from 5:30pm-6:30pm Monday thru Thursday.
